MediawikiでInternal Server Error発生

いつのバージョンからだか分からないけど、Mediawikiで画像を保存すると表示しなくなり、また画像ファイルを参照するとInternal Server Errorという表示になり画像が表示しなくなる事象が発生したので、その原因と対策をまとめてみた
